anytime between February 1, 2011 and March 31, 2011.
2. Enclose a clear copy of your original, dated sales receipt. Keep the original
for your records.
3. Enclose the original UPC code cut from the bottom of the package. UPC
codes that are handwritten or mechanically reproduced will not be
accepted.
